HMS is a Collaborative Quality Initiative funded by Blue Cross Blue Shield of Michigan and Blue Care Network (see www.valuepartnerships.com). The data-driven collaborative is made up of hospitals across the state of Michigan. The goal of the consortium is to improve the quality of care for hospitalized patients who are at risk for adverse events. The initial focus will be improving the care of medical patients at risk for hospital-associated venous thromboembolism (VTE). Adverse events related to VTEs are common and problematic, leading to poor clinical outcomes and prolonged hospital stays. HMS is targeted at working directly with Hospitalists. Hospitalists, due to their management of diverse groups of hospitalized patients and close interactions with hospital staff such as sub-specialists, nurses and pharmacists are ideally positioned to champion quality improvement efforts at preventing adverse events in hospitalized patients.
