[edit] Project Description
The Patient Safety Indicator – Case Review System (PSI-CRS) enables identification of inpatients who acquired a medical condition after hospital admission. It sends information about each patient to clinicians who analyze the condition and determine if the problem was preventable. The PSI-CRS enables the University of Michigan Health System (UMHS) to take corrective action and improve patient safety.

- Identify inpatients with a condition acquired after hospital admission and refer cases to the appropriate clinician for review.
- Provide more accurate results than U.S. government indicators, which identified false positive cases.
- Prepare UMHS for new federal laws that will reduce Medicare reimbursement for hospital-acquired complications of care.
[edit] System/Application & Data Sources
The system uses data that are a by-product of the Hospital billing process. These data include codes of patient diagnoses, some which describe complications of care. To identify potential patient-safety events, UMHS applies to this data a set of algorithms which are part of the Patient Safety Indicators developed by the U.S. Health & Human Services, Agency for Healthcare Research & Quality (AHRQ).
[edit] Project Purpose, Scope, & Deliverables
Design and pilot-test a system that:
- Identifies and analyzes otherwise unreported adverse events using readily available hospital data
- Improves on existing error reporting systems.
- Incorporates a diagnosis timing variable to exclude cases where inpatients have a condition acquired prior to hospital admission.
- Captures information about the preventability of these events to facilitate interventions and quality improvement in patient care.

Note: The PSI-CRS has the potential for commercial development and licensing to other acute care hospitals.

UMHS has pilot-tested the Patient Safety Indicator – Case Review System, and it plans to implement the system across U-M inpatient facilities for use by UMHS faculty and staff who are involved in clinical care and quality improvement.
